The first step in treating lacunar stroke is to determine the cause. Researchers used genetic analyses to compare the DNA of 7,338 lacunar stroke patients to that of 25,4798 healthy individuals. The researchers found several genetic changes in patients with lacunar stroke, including a gene that makes small blood vessels "leaky" and more susceptible to toxicity. These changes also may affect the way messages travel around the brain. Ultimately, they hope to conduct clinical trials within the next 10 years.

If a person experiences symptoms of a lacunar stroke, a doctor will evaluate their medical history. He or she will also check vital signs and perform an electrocardiogram. CT scans and magnetic resonance imaging can also be used to diagnose a lacunar stroke. A CT scan, however, is not very sensitive to detect lacunar strokes. An MRI, on the other hand, is more sensitive and can reveal blockages in the blood vessels.

Although lacunar stroke is usually undetected, it should not be underestimated. The symptoms of lacunar stroke vary depending on where it occurs. The absence of blood flow to smaller arteries is caused by chronic high blood pressure. Similarly, cholesterol plaques and blood clots can build up in these arteries, preventing blood flow to vital parts of the brain.

A large majority of experimental models focus on the pathophysiology of cortical ischemia. Nevertheless, the pathogenesis of progressive motor deficits is not completely understood. Modern neuroimaging techniques are helping to fill this gap. However, the clinical trial data presented are far from definitive. In the future, we may be able to identify underlying mechanisms for progressive hemiparesis. If we know more about the pathophysiology of lacunar infarction, we will be able to better treat the disease.

If the symptoms of lacunar stroke are severe, people may need intravenous clot-busting drugs. Other treatments may involve direct implantation of medications in the brain. If the clot or blockage is severe enough, a neurosurgeon may remove it. Although lacunar stroke can lead to permanent disability, early diagnosis can greatly improve the odds of survival. With the proper treatment, patients can often return to their usual routine and resume daily activities.

Patients with lacunar infarction need to be monitored and managed by a team of health care providers. They need to see a neurologist as soon as possible, and their physical therapists should be involved with their care. Rehabilitation therapy is necessary to restore neurologic function. Primary care providers are also responsible for long-term follow-up. If a patient is suffering from a cardiovascular risk factor, intensive antihypertensive therapy must be implemented. Additionally, patients must monitor their blood glucose levels to prevent further stroke.

One limitation of this study was the lack of a control group. A more age-matched comparison would have revealed the influence of acute lacunar stroke on cognitive abilities. It is important to note that patients with single symptomatic lacunar stroke have a better prognosis than patients with silent lesions. Further, patients with multiple lacunar stroke may be more likely to develop dementia. There is still no definitive treatment for lacunar stroke.

It is important to recognize the signs and symptoms of these strokes, as they are often unidentifiable without imaging studies. It is also important to note that hypertension and diabetes mellitus are major risk factors.
